Team awards presented to the top two teams in
each division.

Entry fee of $10.00 per player minimum of 5 players
required.

Registration deadline: August 22nd

Limit of 10 teams per age division.

* Players under the age of 18 must have the
  parents signature to play

* Games will be played 4 on 4 plus a goalie

* Tournament will be open to both male and 
  female

* Games will feature two 15-minute stoptime 
  halves 

* Games will be played according to the 1997-
  1999 USA Hockey In-Line offical playing 
  rules, including no-offsides, no illegal
  clearing and no body checking.

* Players will be required to wear: Hockey
  helmet, gloves, elbow, knee, and shin pads.
  IF YOU DO NOT WEAR THE REQUIRED EQUIPMENT
  YOU WILL NOT BE ALLOWED TO PLAY!

* Strongly recommended is protective cup,
   mouth guard, full face shield, protective
   pants (girdle).

  of 18, he or she will need a parent's or guardian's
  signature to play. You may photo copy the waiver
  section of this form if additional copies are needed.







   In consideration of your acceptance of my entry,
 my parents, heirs executors , including to be legally
 bound, waiver and release the City of Kewanee,
 Kewanee Chamber of Commerce, Kewanee Hog Day Festival
 committee, Moose Lodge #1261, Kewanee Hockey Association,
 and any co-sponsors and their respective representives,
 and all others directly or indirectly involved with this
 In-Line Hockey Tournament, from all claims of rights to
 damages for injuries or losses suffered by me directly
 in training for, traveling to, and or from copeting in
 or attending the KEWANEE HOG DAYS '97 ROLLER HOCKEY
 TOURNAMENT.



 PARTICIPANTS MUST PROVIDE THEIR OWN MEDICAL INSURANCE
